40多歲的張先生因工作天天應酬喝酒逾20年，今（2024）年農曆年前的1次聚餐幾杯酒下肚，上腹痛到坐立難安，還噁心翻胃，就醫診斷為「酒精性胰臟炎合併胰管結石」胰管內有1顆約1公分的結石，經轉送到中國附醫接受「膽胰管內視鏡碎石」治療，90分鐘就不痛且復原出院，他也開始認真戒酒。

中國附醫內科部消化系副主任黃文信表示，胰臟炎是胰臟的發炎性疾病，分為急性和慢性，急性胰臟炎多表現為突發性腹痛，且痛感持續並會傳至背部；慢性胰臟炎則與急性胰臟炎的反覆發作有關，酒精是造成慢性胰臟炎及胰管結石的主要原因之一，慢性胰臟炎的其他常見原因還包括長期酗酒、新陳代謝疾病或先天性膽胰管構造異常。

在台灣，約有一半的慢性胰臟炎病人會產生胰管結石，這些結石阻塞胰管，導致慢性腹痛或反覆性的胰臟炎發作。黃文信指出，正常的胰管直徑小於3毫米，而張先生的結石直徑達到1公分，如採傳統內視鏡取石變得非常困難。

以往治療胰管結石的方式是使用內視鏡將取石網或碎石器伸入胰管內嘗試將結石絞碎後取出。但面對較大的結石，內視鏡取石網常無法有效取出，最終只能置放胰管支架引流胰液，有時需要長期反覆地置放支架，嚴重者甚至需進行外科手術。

如今，膽胰管內視鏡的應用使治療更為精確有效，透過直徑僅3.5毫米的內視鏡導管，可以經由十二指腸鏡進入胰管，使用雷射或震波技術碎石，無需開刀即可順利取出胰管內的結石。自2020年以來，中醫大附醫已成功執行數10例膽胰管內視鏡「震波碎石」手術，積累了豐富的臨床經驗。
